I/We hereby authorize Iron Bar Fitness (the "Payee") to debit my/our account at the financial institution identified above for the purpose of recurring membership dues in the amount of $28.00 bi-weekly (plus applicable taxes), or such other amounts as may be agreed upon in writing.
This authorization is to remain in effect until cancelled in writing by me/us, with at least 30 days' notice, or upon termination of the membership agreement, whichever occurs first.
I/We acknowledge that this authorization is provided for the benefit of the Payee and the financial institution, and is provided in consideration of the financial institution agreeing to process debits against my/our account in accordance with the Rules of the Canadian Payments Association.
I/We have certain recourse rights if any debit does not comply with this agreement. For example, I/we have the right to receive reimbursement for any debit that is not authorized or is not consistent with this PAD Agreement. To obtain more information on my/our recourse rights, I/we may contact my/our financial institution or visit payments.ca.
I/We warrant that all persons whose signatures are required to authorize transactions on the account identified above have signed this authorization.
The Payee agrees to provide at least 10 calendar days' notice before changing the amount or timing of the PAD, unless the change is requested by the Payor.
